The births were between 1942 and 1962, during that time she had 18 children with just 1 set of twins!

Depending on what timescale we are talking about, I suspect that it all depends on what information the registrar was given. Perhaps the mother simply "implied" that she was married to the father of the children by giving his surname and her "former" name, or perhaps they did marry and when the split up she reverted to her former name?

The birth certificate would show whatever information she gave to the registrar.

Edit- Mum went on to have several more children with her partner whom she never married, but they are all registered with just his surname, her married name didn't arise again. Why would that be?

Later after having her children she went on to marry a different man and her surname was still shown as that of her original marriage.

That would be the way the births were registered if the father of the last 4 children was not the woman's husband at the time or if she was no longer married to her husband.

The births would be registered under the married name as that was still her surname whether or not she was still with her husband. The biological father must have gone with her to register the births or signed a declaration admitting paternity for the children to be registered with his surname too.

I wonder if you can help please. I'm trying to help a friend who comes from a large family piece together the birth information of his siblings.

The mother married and had 1 child registered by the married surname. However, the next 4 children appear in the birth indexes under the married surname and again under a different surname, the second surname being that of what we believe to be the biological father.

Is this scenario commonplace in your experience? I have deliberately not mentioned names to protect privacy.
