Central Islip State Hospital was a psychiatric hospital.

If that's the correct man, he was obviously a long-term patient - hence Ethel's possible face-saving as ErikaH suggests, as mental illness wasn't talked about in those days..

Harry Nowack seems to have disappeared from the scene between c1910 and 1915, when Ethel and her children are living with her parents.

Now looking for m of Ethel b 1888 census said 1906 to a Harry Nowack/Novak/ Novick (all different spellings) he was b 1884 re census, they had 2 children Rose/Rosie 1908 & Jacob 1911 of Bathgate Ave Bronx NY
Have try all spellings but no joy

Did the children you know about all remain in New York? Widowed mothers often go to live with one of their children, which can sometimes take them away from their home area.

Harry did die in 1946 in the Bronx NY (from SSN), can find no death for his wife, have tried all her used names, i would say she died in 1950s, no records in Bronx or any New York cemetery

In his OP, Ian said Harry died 1946 - but I can't see any record of that.

Four trees on Ancestry claim date of death was 25 March 1946 - but no place or proof.

None of them has a date of death for his wife.

There would have been no legitimate births between 1902/3 and 1905

Do you know when Harry and/or his wife died? The obit might hold useful details
