Someone else double check the following please.

On the first page
Under the top section, “For events registered in England and Wales”-
Tick Death Certificate.
Age of death in years – 50
Is the GRO index reference number known – yes
Year in which the event was registered – 2005

On the next page (2005 has been pre-filled)

Surname of deceased – (enter Either Bayliss OR Mitton but not both)
Forename of deceased – Jennyfer
District name and number – Cirencester 480/1
Entry number – 39 (corrected)
DOR – 03/2005

Everything else should be straight forward. Unless your marriage is imminent, the standard delivery should suffice. Assuming they don't have a back log, their ETD (2nd class post) used to be 4 working days after the order was placed eg next Friday.

Express delivery would be sent out more quickly.

Tick the box that asks if the GRO ref is known then use the ref in the boxes where asked

Thats all you need to fill in ,apart from her name of course and that its a death cert you want

Good find KenSE. Even if the divorce didn't go through, Peter is definitely a Widower. Buy the certificate and a copy of your marriage certificate if you don't have it. That should cover you.

The death is also reported under the surname Mitton, same details.

We think that Jennyfer died in 2005. This is the death reg you need to order the certificate in the hope that it will confirm it is the correct person. The informant may be know to you.

To check the status of your and Jennyfer's marriage, a quote from the National Archives.....

Decrees absolute (1858 to present)
Find the decree absolute to obtain legal proof of a divorce in England or Wales by requesting a search (£) of the Central Index of Decrees Absolute. To do so, download Form D440 from Her Majesty's Courts and Tribunals Service website and post it to the Principal Registry of the Family Division."

BTW, anyone know why my post of 1 May @ 15.59 was reported? As far as I can remember it didn't give any details which would have breached anyone's privacy.

Edit at 1756 - the post has been restored.
"A Jennyfer Bayliss was born in 1954 - is this the mother you are referring to?
She might have married in 1979 to a Shaw
On the other hand there is a death recorded which matches with the birth under the surname of Bayliss in 2005."

The use of potential espousal surnames is vital when attempting to trace people, especially when we are working with limited information.

Might one remind the Reporter that most members who try to help are more than willing to edit posts if the Reporter believes them to breach the T&C. They only need to ask!

Do you know the name of the daughter? When and where was she born? Was Jennyfer married at the time, and did the father acknowledge paternity? If he did, his surname might be on her birth reg.

We also need to know why you wish to find the daughter

Right - found a tentative connection to Peter.

Jennyfer may have married a Mr Mitton in 1986. Still can't find any Mitton/Bayliss children!
EDIT- scrub that if Reggie is right to say there are no SHAW or MITTON connections

As Reggie says, who registered her death?
