The word Formerly is what is used on birth certificates to describe the maiden name of a mother. |

Thanks Kath, are you saying that the word Formally would not be on a certificate, as someone has said it would and if they used formally then it was an alias? |

I would say that whoever used the word formally on a certificate just mis-spelled formerly. |

I agree with Margaret. The use of Formally on a certificate would just be a mis-spelling of Formerly. |

Long shot, but formally could also mean the proper name of a person, e.g. my g aunt Polly was married with the name Polly, but the cert said "formally Mary". |
