Birth Register - Help Please

I have a copy of my Aunt's certificate of Registry of Birth:

Margaret Helen Pheby, born 20th August 1912, has been duly registered by me at entry No 355 of my Register Book No 11a
District: Maidenhead Sub-District: Maidenhead.

No name of the mother.

I have checked the births for September 1912 and Margaret Helen Pheby is listed, in the district of Maidenhead but it is Vol 2c Page 766 - are they the same person, and could someone please explain why there is a difference, or did you have two forms?
