Army Records for those who served or finished serving after 1920 are not available on line or at TNA at Kew, they are only available to next of kin. You’ll need to apply to the MOD:

If you phone the number on the same website, they will send you an information pack which includes the application form.

UPDATE - I've just tried, the site seems to be down at the moment, try again later.

I am trying to get the form from this webpage http://www.veterans-uk.info/service_records/service_records.html but it is not working.
As my grandad has given me permission to obtain a copy of his army records, he served between 1954-1978,
Is there anyother way i can get the form so i can get it sent off,.
